Yesterday's proceedings were opened for the purposes of issuing a death certificate only, he said.

The matter was still under investigation by the Health and Safety Authority and it was anticipated a file would be sent to the Director of Public Prosecutions in the next few weeks, Garda Supt Michael Maher said.
