In additional time Kilkenny struck in the 71st minute when Eddie Brennan's goal made it 2-12 to 1-12.
The visitors sealed victory when they scored 2-1 in the last three minutes. John Hoyne notched a goal before Martin Comerford also scored.
Offaly led by 1-6 to 1-4 at half-time; Kevin Kelly scored Offaly's goal while Charlie Carter netted for Kilkenny.
