Two investigations were launched in Derry yesterday following the death of a man who was found lying on a footpath shortly after he had left Altnagelvin Hospital where he had been an in-patient.
The circumstances in which the man, believed to have been in his early 40s, left the hospital were investigated by the hospital authorities. The office of the Police Ombudsman started a separate investigation because the man died in the presence of police officers who had earlier attempted to resuscitate him.
The man left the hospital late on Sunday night and was found in the early hours of yesterday morning lying on a footpath at Church Brae, about one mile from the hospital.
Local people informed the emergency services and when police officers and ambulance medics arrived on the scene, they attempted to resuscitate him. He died later in hospital.
Locals said he was seen dressed in his pyjamas, walking up and down Church Brae, shortly before he collapsed.
