Child and Family Agency boss says Hiqa has been notified following the incident

Tusla, the Child and Family Agency, is to carry out an investigation following the death at the weekend of a young person in its care.

Chief executive of Tusla Fred McBride said: “The loss of any young person is a tragedy for the whole community. I would like to offer my deepest sympathies to the family and those who knew him.

“Tusla is offering support to his family and to anyone impacted by his death through the local social work office.

“Where a death occurs of a young person in the care of Tusla an internal review is undertaken in line with established procedures,” he said.

Tusla said the case has been notified to the National Review Panel and Hiqa in accordance with protocol.

The agency would not confirm the identity of the young person in question.
